having trouble with erros

I almost have a small e-card site up, for an elementary school I work for, but for a few errors in code. Maybe one or two lines! I need help. Can someone help me? I'll email the small php files to you and you can take a look. the form writes to a mysql database no prob. I'ts the way back...nothing. Thanks.

if they arent too big just post them in succession as part of your forum post, that way we can all take a detailed look at them...
