Add/Remove text box

I'm looking for a way to add extra fields to a form ( like a text box) if a user clicks an 'add' button and remove the last added text box if the user clicks the remove button.

I'm new to javascript and am hoping someone can help we with this.

Also, will I be able to access the text in these text boxes via php once the form is submitted?
