Javascript User input prompts

Hi There,

I am working on a current project and need some assistance please

I have a page called CaseNote i need to create a button that open a series of JS user input prompts such as

• Customer's Name

• Customer's DSL Line

• Customer's Username

• Customer's TAN

Then i need this information put into a textbox on the page called innerhtmlTest

Once that is done a simple Spell Back & a second prompt that says Sure "Customer's Name" i can help you with that
