how to link radio button with other radio buttons and text fields

i have created a form with some radio buttons and some text fields.. example shown here :

O resume received
O resume evaluated
O call him for phone interview

option 2
O Interviewed by Phone
O phone interview form completed
O call him for face to face interview
O rejected
reason for rejection |_________________________|

option 3
O Conducted Face to face Interivew
O personal interview form completed
O call him for technical interview
O rejected
reason for rejection |_________________________|

option 4
O Make offer
O offer accepted
O offer rejected

O date of joining


What i want is that if resume recieved is not click. . then none of the radio buttons from there on get active/able..means grayed out.

If recieved and evaluated are clicked (yes) then i will manually clik on call him for phone interview... then the option 2 get activated.. not 3 and not 4
even if rejected in phone interview then option 3 and option 4 should get not active (they should be grayed out).

same is for the rest... if manully click on call him for face to face interview then the option 3 get active not 4. even if rejected in face to face interview the option 4 should get not active... Similary if manully clicked on call him for technical test.. then other options get highlighted /active up. Same way in option 4 we make him offer, if accepted then the date of joining orother fields related to this get active up.

please help me in this...

i feel there no such way of controlling the form, for the members are not replying to my query. May be have designed it wrong way, can you tell me what is the exact way of doing the same...
