MySQL and PDF forms
I know this might be much but maybe some one can direct me where to go.
I have a group of physicians with id for different insurance companies. I have a form that is in Adobe on our intranet. What I need to do is when someone needs to use this form all need to do is put the physicians name in and what insurance company form needed then input that into the form. I am sure that this sounds like a lot but again if someone could point me in the right direction I would apreciate it. |

Well, the easiest thing for you to do will be to get a licese for PDFLIB complete with PDI functionality. With this, you can very easily open a template PDF (the blank form) and write text on top of it using functions provided in PHP. The downside is that the license for PDFLIB is a couple of grand.
