Hello guys...!
CAPTCHA stands for "Completely Automated Public Turing test to tell Computers and Humans Apart"......The idea is to create a test that any human can pass easily but computer programs will fail....!

Which kind of CMS you are using. In wordress just install captcha plugin. In others just get the captcha's code and put into your backend. Its simple...

There are many tutorials that could help you with this.
You may just use a simple HTML code with forms along with security verification that asks the user to verify whether they are human.
You can also do it with PHP and AJAX.

Whether you’re using CMS or not, there a lot of free captcha plugin across the web. Just download the file and include it in your code. Look for documentations on how to use the plugin to make your work fast.

When we say CAPTCHA, it is a validation tool that requires a person to type in the text that they see in graphic in order to submit a form and it is used to confirm that the form is coming from a real person. You will need to sign up for the reCAPTCHA service and get an API key.Once you've done it, download the reCAPTHCA library. Extract the zip file and place the file recaptchalib.php in your website.

How to Add a Captcha to my website ?

you must need a basic understanding on PHP GD and session handling. I found this on google, it's really easy to follow: codewalkers.com/c/a/Miscellaneous/Creating-a-CAPTCHA-with-PHP/

How to Add a Captcha to My Site ?

Add the following PHP in your processing page:

require_once('recaptchalib.php');
if (isset($_POST["recaptcha_response_field"])) {
$resp = recaptcha_check_answer ("private_API_key",
$_SERVER["REMOTE_ADDR"],
$_POST["recaptcha_challenge_field"],
$_POST["recaptcha_response_field"]);

if ($resp->is_valid) {
# they got the captcha right
echo "Do something here, such as setting a variable you can use later.";
} else {
# set the error code so that we can display it
$error = $resp->error;
}
}

Change private_API_key to the one you received when you signed up.
To display the captcha:

Add the following PHP in the page that will display the form:

require_once('recaptchalib.php');
echo recaptcha_get_html("public_API_key", $error);

Change public_API_key to the one you received when you signed up.
