Looking for a web host

Generic title I know. I've been looking for a web host with the following characteristics:

1. Under $10.00 a month, no setup fee. $5 to $7 would be the best range but I'm willing to pay more if the option arises.
2. Has decent support - I may be adding a blog or installing imageboard software, and if I mess something up I'd like some help fixing it.
3. A lenient AUP/TOS.
  • I do not plan to host adult content on my site, however, since I'm adding an imageboard that sort of thing may pop up inadvertently. While all users will be responsible for their posts, I do not want to be shut down because some idiot posted something against my AUP.
  • In addition, I will probably be storing files on the site that may or may not be linked to in pages (I do some level design and occasionally have to hand out beta versions of levels to volunteers. Obviously I do not want to link to half-finished maps publically.)
4. Unlimited MySQL databases if possible.

Bandwidth isn't too much of a concern, I don't plan to get a large amount of traffic, but I'd like some room for error too. Storage is the same way, a gig would be nice, I don't plan to use any more than that, but of course bigger is better.

I've found a few that fit two or three of these criteria but none that have fit all. Any help would be appreciated.

Planetonline.net is the best in its field with many years of experience. Personally my big brother had hosted a website from planetonline and after that he is just enjoying that.So my suggestion is u also have to try planetonline.

I'd use up 50mb in less than a day. A standard level pack is over that much simply by itself, not counting all images on the site and the imageboard software. Aren't providers supposed to be OVERSELLING disk space? In addition, according to their ToS, they do not permit adult content, and they can charge you a $500 "cleanup fee" at will if they decide you violated their policies. Which means if someone posts something they shouldn't I could get slapped with a huge fine and have my entire site shut down on a whim. I don't think they'll work, sorry.
